1. Treatment Description

This agreement is between the client (hereinafter referred to as “the Patient”) and Awari Health (hereinafter referred to as “the Practice”), located in Hobart, Tasmania, Australia.

The Patient will receive treatment for pain reduction according to the Awari Health method. The Awari Health method is a pain therapy method that focuses on relieving tension in muscles and fascia to reduce chronic pain. It involves targeted stretching/foam rolling, osteopressure, and specific exercises designed to restore natural movement and balance in the body. This approach is non-invasive and aims to address the root causes of pain rather than just the symptoms. This is an alternative therapy approach and not a medical treatment.

2. Risks and Disclosures

The Patient acknowledges that they have been informed of and understand the possible risks associated with the treatment, including but not limited to:

Initial aggravation of symptoms
Formation of pressure sores
Fatigue and Tiredness
The Patient understands that all potential risks may not be known or listed here.

The Patient is aware that this treatment is not a medical service and does not replace the need for medical care. The Awari Health method is not covered by health insurance and is provided as a self-pay service.

3. Patient Responsibilities

The Patient agrees to:

Inform the therapist of all known physical and medical conditions, medications, and injuries prior to each session.
Keep the therapist updated on any changes in their physical health, medical conditions, or medications.
Inform the therapist immediately if they experience any discomfort during the therapy session.
Carry out the exercises practiced during the treatment appointments independently, regularly, and consistently in daily life, as this is necessary for the treatment’s success and maintenance of health.
Understand that the Therapist may end the session for any inappropriate behavior.

4. Payment Terms

Treatment fees are as specified on the Awari Health website.
A typical session lasts between one hour and one and a half hours.
The Practice recommends three treatment appointments per area of pain as a general rule.
Please note that only one discount code can be applied per purchase.

Credit card booking fee is non-refundable

5. Cancellation Policy

The cancellation policy is different depending on the type of session you book.

  • For single session bookings (Swift & Pressure):
    • Free cancellation or rescheduling if done 24+ hours before appointment
    • 50% cancellation fee if cancelled within 24 hours
    • Credit card booking fee is non-refundable
  • For multiple session bookings (Emersion):
    • Can cancel after the first session
    • Pay full price for the first session, receive refund for remaining sessions
    • After first session, rescheduling allowed but no cancellations
    • Option to gift unused sessions to a loved one
    • Credit card booking fee is non-refundable

6. Liability Waiver

In consideration of being permitted to participate in the Awari health method treatments at Awari Health, the Patient agrees to assume full responsibility for any risks, injuries, or damages, known or unknown, which might occur as a result of participating in the program. The Patient acknowledges being informed of possible consequences that may arise from the treatment, such as initial aggravation and the formation of pressure sores, and other potential discomforts even though no other risks are known at this time.

The Patient understands that this treatment is not a medical service and does not replace the need for medical care. The Patient accepts responsibility to inform the therapist of any discomfort during therapy and request pressure adjustments as needed. The Patient has notified the therapist of all known physical and medical conditions, medications, and injuries and accepts responsibility to keep the therapist updated on any changes to their physical health and medications, with no liability on the therapist’s part should the Patient fail to do so.

For successful treatment and maintained health, the Patient acknowledges the necessity to perform the prescribed exercises independently, regularly, and permanently in daily life. Typically, 3 treatment appointments are recommended per pain area.

Client hereby assumes full responsibility for receipt of the therapy, and releases and discharges Awari Health and its therapist from any and all claims, liabilities, damages, actions, or causes of action arising from the therapy received here-under, including, without limitation, any damages arising from acts of active or passive negligence on the part of the Therapist, to the fullest extent allowed by law. This includes any use of the facilities at Eternal Dance studio, its members, therapists, staff, and all other personnel.

7. Informed Consent

By signing this agreement, the Patient acknowledges that they have been informed of the nature of the Awari Health method treatment, its risks and benefits, and have had the opportunity to ask questions. The Patient gives their informed consent to receive treatment from Awari Health.

8. Legal Clauses

Governing Law: This agreement shall be governed by and construed in accordance with the laws of Tasmania, Australia.

Severability: If any provision of this agreement is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

    3. Cookies
      If you leave a comment on our site you may opt-in to saving your name, email address and website in cookies. These are for your convenience so that you do not have to fill in your details again when you leave another comment. These cookies will last for one year.
      If you visit our login page, we will set a temporary cookie to determine if your browser accepts cookies. This cookie contains no personal data and is discarded when you close your browser.
      When you log in, we will also set up several cookies to save your login information and your screen display choices. Login cookies last for two days, and screen options cookies last for a year.       With medical professionals directly involved in the patient’s care

    6. Data Retention
      If you leave a comment, the comment and its metadata are retained indefinitely. This is so we can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.
      For users that register on our website (if any), we also store the personal information they provide in their user profile. All users can see, edit, or delete their personal information at any time (except they cannot change their username). Website administrators can also see and edit that information.
      For treatment-related information, we retain patient records for a minimum of 7 years from the date of last entry, or as required by Australian law.

10. Acknowledgement and Agreement

By booking a therapy service with The Practice, whether through website of The Practice or by any other means, The Patient acknowledges that they have read, understood, and agree to the terms and conditions outlined in this liability waiver and informed consent document. This agreement automatically comes into effect upon completion of The Patient’s first booking.
The Patient’s booking of our services constitutes your electronic signature and acceptance of this agreement in its entirety. The Patient confirms that they have had the opportunity to review this document and seek independent legal advice if desired.

The Patient agrees to not book the services of The Practice if they do not agree with these terms.

The Patient agrees to contact The Practice prior to making a booking, if they have any questions or concerns about this agreement, they agree.
